โดยการสมัครทางอินเตอร์เน็ตนี้ ข้าพเจ้าขอรับรองว่า ข้อมูลและรายละเอียดที่ระบุข้างต้นเป็นความจริงและถูกต้องทุกประการ ข้าพเจ้าตกลงส่งมอบเอกสารที่เกี่ยวข้องกับการขอสินเชื่อนี้และตกลงจะไม่เรียกคืนเอกสารทั้งหมด และข้าพเจ้ายินยอมมอบอำนาจให้บริษัท บัตรกรุงศรีอยุธยา จำกัด (?บริษัทฯ?) ทำการตรวจสอบ เปิดเผยหรือแลกเปลี่ยนข้อมูลส่วนตัวของข้าพเจ้า กับข้อมูลที่มีอยู่ในฐานข้อมูลของบริษัทฯ บริษัทในเครือ บริษัทข้อมูลเครดิต สถาบันการเงินต่างๆ บริษัทซึ่งเป็นผู้ให้บริการบริหารความเสี่ยง หรือบริษัทที่ประกอบธุรกิจเกี่ยวกับการจัดเก็บหรือประมวลผลข้อมูล ไม่ว่าจะเพื่อประกอบการพิจารณาคำขอรับสินเชื่อของข้าพเจ้าและผู้สมัครบัตรเสริม หรือเพื่อประโยชน์ในการประเมินเครดิตของข้าพเจ้า หรือเพื่อประโยชน์ในการเสนอบริการ หรือสินค้า หรือการดำเนินธุรกิจของบริษัทฯ หรือบริษัทในเครือ หรือในการรับทราบบริการประกันที่บริษัทฯ เห็นว่าเป็นประโยชน์แก่ข้าพเจ้าและผู้สมัครบัตรเสริม หรือเพื่อประโยชน์ในการประมวลผลข้อมูลของข้าพเจ้า เช่น เก็บรวบรวม บันทึก จัดหมวดหมู่ เก็บรักษา จัดลำดับตำแหน่ง แก้ไข เลือก เรียก เปรียบเทียบ ใช้ เชื่อมต่อ ระงับ เปิดเผยโดยเจาะจง เปิดเผยโดยทั่วไป รวมทั้ง ลบ ทำลาย ส่ง หรือโอนซึ่งข้อมูลดังกล่าวไม่ว่าด้วยวิธีการใดๆ ซึ่งการตรวจสอบหรือเปิดเผยหรือแลกเปลี่ยนข้อมูลดังกล่าวนั้น ข้าพเจ้ายินยอมให้บริษัทฯ คัดลอก หรือทำสำเนาเอกสารแสดงแหล่งที่มาของรายได้ของข้าพเจ้า รวมถึงการส่งข้อมูลของข้าพเจ้าไปต่างประเทศ (อาทิ ประเทศออสเตรเลีย หรือประเทศอินเดีย เป็นต้น) ทั้งนี้ ข้าพเจ้าตกลงจะไม่เพิกถอนความยินยอมนี้ไม่ว่ากรณีใดๆ และจะไม่เรียกร้องค่าตอบแทนหรือค่าเสียหายใดๆ จากการกระทำของบริษัทฯ หรือบุคคลที่ได้รับข้อมูลโดยความยินยอมของข้าพเจ้า ข้าพเจ้ายินยอมผูกพันตนตามข้อกำหนดและเงื่อนไขการเป็นสมาชิกบัตรเครดิตซึ่งส่งแนบมาพร้อมบัตร เว้นแต่ข้าพเจ้าจะได้ตัดบัตรเครดิต ออกเป็น 2 ส่วนและส่งกลับคืนไปยังบริษัทฯ ข้าพเจ้ายอมรับว่าบริษัทฯ มีสิทธิโดยชอบที่จะปฏิเสธใบสมัครของข้าพเจ้าโดยไม่จำเป็นต้องระบุเหตุผลหรือคำอธิบายใดๆ รวมทั้งไม่จำต้องส่งใบสมัครและเอกสารประกอบคืนให้แก่ข้าพเจ้า
